2011 - 2012 Registration

Registration for the UDA Fall/Spring session 2011-12 is still continuing for classes that have not reached their maximum capacity. Please contact the UDA office to register your child for classes today! Registration for all recital performance classes will close on December 15th.


Forms Required for Registration

Registration Forms must be completed and signed by the parent or guardian deemed the responsible party for the account.

  1. Registration Form - new students only
  2. Medical and Legal Release Form
  3. Media Release Form
  4. Account Contract - available from UDA office.

Fees paid at Registration

  1. Registration Fee - $30 per student/$75 per family
  2. First Month Tuition
  3. Last month Tuition (only 2 weeks, ½ month tuition)
  4. If registering after October 1st - $75 Recital Fee
  5. If registering after December 1st - $35 costume fee per recital routine

Additional registration information:

  • All students are evaluated in the first 30 days of class to ensure proper skill level placement.  Instructors will determine if there is an adjustment that needs to be made.
  • Parents with preschool children MUST bring their dancer to registration to be sized for the PRESCHOOL UNIFORM (approx $40.00). 
  • Parents who wish to order shoes or other apparel at registration should bring their dancer to be sized.

*Registration for Master Classes, Company auditions and other special events are done in the studio office as needed.
